What to do at first visit

  1. Fill up Declaration Form and Part I of the Application for Interbank GIRO Form (obtainable at the registration counter or Baby Bonus website).
  2. Submit the form at the registration counter.
  3. The clinic staff will verify your documents.
What to bring for verification when submitting Interbank GIRO Application
  1. Trustee/Parent's NRIC.
  2. Birth certificate of your child.
  3. Latest Children Development Account Statement or up-to-date online statement of account.
  4. Birth certificate of your child's sibling(s) - only if funds are used by siblings.
What happens after submitting the Interbank GIRO Form
  1. The clinic will submit the completed GIRO application form to the bank. The bank will process the application within 4 weeks of receiving the properly completed form. (Note: Incomplete form may delay the approval of your interbank GIRO application)
  2. Once your GIRO application has been approved by the bank, you can start using your CDA to pay for your children's medical-related expenses. Before your GIRO application is approved, you may have to pay cash for your visit.
What to bring along for subsequent visit
  1. Trustee/Parent's NRIC.
  2. Latest Children Development Account (CDA) Monthly Statement or online CDA Statement of Account.
